The American Academy of Family Physicians EveryONE Project released The Neighborhood Navigator, an interactive tool that helps family physicians address social determinants of health by linking them to thousands of social services in their area. The tool lists over 40,000 social services by zip code including: food, child care, housing, transportation, employment, and legal services.
